We accept most major insurance plans and Medicaid. If an insurance company is not listed, please contact SLDCenter for more information. We always make every effort to accommodate our patients and work with most insurance companies.We also offer payment plans for individuals without coverage and accept most credit cards.

Billing & Financial Policies
We have established our financial policies so that you can be informed and plan accordingly. We welcome you to discuss with us any questions you may have.

Co-payments are due when you check in for your appointment.

Returned checks will result in a $30 fee.

We routinely check your insurance eligibility prior to your appointment. If your insurance company reports that you are not eligible on the day of your appointment, you may need to make financial arrangements prior to your visit.

You are responsible for knowing your insurance coverage. We are not a party to the contract between you and your insurance carrier. Your insurance carrier determines your co-payment and patient’s balance.

We must receive notification from you about a change in your insurance as soon as possible to avoid claim denial, which may result in a bill to you.
